Choose the option that works best for you:

1.) Buy the templates and customize them with your            compliance team for your agency:

• Upon payment, templates will be provided via email and also via snail mail on a cd.
• Documents will be in Microsoft Word format (policies, table of contents, attestation), Microsoft Publisher (training booklet) and Microsoft Power Point (training slides).

Cost - $2,500.00

2.)Buy the packet and we can work together to customize them for your agency:

• HCCE will email templates to one recipient upon receipt of payment of $2,500.
• Up to 12 hours of teleconferencing with a designated compliance officer to customize the packet. (HCCE will teleconference with 1 to 2 staff members only) 
• Email of revised documents to one recipient
• Up to two more revisions of each document
• HCCE will email final documents with final invoice of $2,750.00 
• Snail mail of finalized packet on cd to one recipient

Due prior to receiving initial templates - $2,500.00

Due upon email of final drafts - $2,750.00

Total Cost - $5,250.00
